The word detox gets used the same way for every substance, but what's happening in the body varies enormously depending on what you're coming off. Some substances create physical dependence severe enough that stopping suddenly carries real medical risk, including seizures in the most serious cases.
Others create a powerful psychological pull without much physical danger at all. Knowing which category applies to your situation changes what kind of support actually matters most.

The Four Categories of Drugs We See Most Often
Opioids
Heroin, fentanyl, oxycodone, morphine, codeine, hydromorphone, and medications like methadone and Suboxone all fall here. Physical dependence is real and withdrawal is genuinely uncomfortable, though on its own it’s rarely as medically dangerous as alcohol or benzodiazepine withdrawal. The bigger danger with this category is often what happens after a period of reduced tolerance, since a return to a previous dose carries serious overdose risk.
Alcohol and Sedative Like Substances
Alcohol, benzodiazepines, and GHB belong in this category, and it’s the one that carries the most serious physical withdrawal risk of any substance we treat, including seizures and, in the case of alcohol, a potentially fatal complication called delirium tremens. Medical supervision for this category isn’t a precaution, it’s a requirement.
Stimulants
Cocaine, methamphetamine, and MDMA fall here, along with prescription stimulants like Adderall or Ritalin when misused. Physical withdrawal danger is generally lower than the two categories above, but the psychological experience, the crash, the cravings, and in some cases paranoia or agitation, can be intense and is a common reason people relapse without support.
Cannabis & Hallucinogens
Cannabis and substances like DMT carry the lowest physical detox risk of any category here, with no known lethal overdose and generally mild physical withdrawal. That doesn’t mean there’s no real dependence to address. Psychological dependence, and in DMT’s case a compulsion tied to the intensity of the experience itself, can be just as disruptive to someone’s life as a physical addiction, even without the same medical urgency.
Signs You Likely Need a Drug Detox with Medical Support
- You've experienced withdrawal symptoms, shaking, sweating, nausea, anxiety, when you've tried to cut back or stop before
- You've had a seizure, or come close to one, during a previous attempt to quit
- You're using more than one substance regularly
- You've tried to quit on your own before and it didn't hold
- You feel physically unwell or unable to function without using

Choosing the Right Length of Your Inpatient Program
Not everyone needs the same amount of time. Some clients are ready to transition out after 30 days. Others, particularly those with a longer drinking history or additional mental health concerns, benefit from 60 or 90 days of continued structure and support. Program length is a clinical decision made with you, not a package you pick off a price list before anyone’s met you. Our admission team will suggest starting with a minimum of 30 days, and deciding to extend once you’ve been in treatment and can discuss your needs with our clinical team.
